Nikolai is exactly right... Since he says he is in 3600 rate centers he mostly like has a agreement with a large CLEC or CLECs who are providing his dids and minutes and then upselling them for profit.
In the issue of quality it depends if they route the media through their servers or not and if they have the amount of bandwidth necessary to carry the traffic.
