You have to keep in mind that origination (from the PSTN) and
termination (into the PSTN) can be -- and often are, in the VoIP world
-- be two entirely independent facilities. And some companies that do
both are only good at one, and so on.
For US48+then some termination, I've generally been very happy with
Junction Networks (www.jnctn.net). For US origination, I recommend IP
Communications (www.ipcomms.net).
